what happens now if i press cart - it goes to homepage not it own page
then the form displays funny like first --> addres
then on another row lastname --- address
that seems not correct.

make sure you create checkout and cart menu items, even if those pages are not accessible from your menus.

I realised I did not correctly answer your question.
The order of the fields is set in the Easy Checkout extension.
However, these may be overridden by J2Commerce's layouts.
Go to J2Commerce -> Setup -> Configuration -> 'Checkout Layout' tab.
You can erase data in the first 2 fields to get the default layouts from Easy Checkout.
You can even make your own layout in those fields to match exactly what you are looking for.
